Isolation can lead to the worsen and/or development of physical and mental health issues. It can also increase family concerns for those who don’t live nearby or who are dealing with their own family issues. Geriatic care managers, can mitigate some of these challenges and help alleviate family concerns with a variety of services. These include:
- Monitoring loved loves for signs and symptoms of worsening conditions
- Advocating & communicating issues if facilities are not allowing visitors.
- Ensuring that loved ones have needed groceries, healthcare supplies & medications
- Running errands
